MASSAGE & IV NUTRIENTS: Scheduling for massage and IV Nutraceuticals are now available for self-scheduling. If you are registering for a new account with Optimal Healthcare, please click on the "New Patient Registration", complete the registration form, and then proceed to "Appointments" tab on the portal. This will allow you to find an available appointment time. Submit your request.

FORMS & QUESTIONNAIRES:
Forms/Questionnaires are requested for consultations with providers and must be completed prior to designated times before your appointment. All consent forms must be reviewed and signed in advance of the appointments. Your appointment may be rescheduled if the forms are not completed. If Consent forms need to be signed in the office, this may take away from the length of your consultation.

First Appointment: Medical Questionnaire, Social History Questionnaire,Medical Symptoms Questionnaire, Family History, and 3-Day Diet Journal. All Consent Forms.

Follow-Up Appointments: Follow-Up Questionnaire, Medical Symptoms Questionnaire, and 3-Day Diet Journal.

Additional Questionnaires may be requested for specific conditions (e.g., Hormones, Thyroid, etc.).


CANCELLATION POLICY:
Cancellation Policy: We have a 72 hour cancellation policy for our appointments and request you be respectful of this. Failure to provide sufficient notice may lead to a 50% charge for the scheduled appointment.
