Forms/Questionnaires are requested for consultations with providers and must be completed prior to designated times before your appointment. All consent forms must be reviewed and signed.
First Appointment: Medical Questionnaire, Social History Questionnaire, Family History Questiionnaire, Medical Symptoms Questionnaire, and 3-Day Diet Journal.
Follow-Up Appointments: Follow-Up Questionnaire, Medical Symptoms Questionnaire, and 3-Day Diet Journal.
Additional Questionnaires may be requested for specific conditions (e.g., Hormones, Thyroid, etc.).
Cancellation Policy: We have a 72 hour cancellation policy for our appointments and request you be respectful of this. Failure to provide sufficient notice may lead to a 50% charge.
